With the surgically repaired wrist and the recoil-sensitive factor, I would suggest that you go with something with less recoil than the .45 / .410 Judge. I have one and believe me it has considerably more recoil than my TCP 738 and my PT138.

First of all, the Public Defender is built on a modified M85 frame whereas the Judge is built on a modified Tracker 44 frame, ergo it is extremely unlikely that barrels are interchangeable. Furthermore, attempting to replace the barrel would be something best left to a qualified gunsmith and would void the warranty on your PD.
At 7 yards the majority of pellets stay on a B-27. At 3 yards it's a nice tidy pattern about the size of a cantaloupe. Although the patterns are of similar size, I can definitely say the longer barrel keeps a tighter group. As far as 410 loads go, I've found the best patterns with the Federal 410 000 buck.Bud's still seems to have some with the 4" barrel, but with the 3" chamber, for under $500. At 25 feet or less has 3-5 inch spread. It puts 5, almost 40 cal holes in a target with each squeeze of the trigger. The 22 mag put bullets through paper sideways, keyhole at 20 to 25 yards with every brand of ammo. 22 long rifle cylinder didn't keyhole as bad but wouldn't group. There is just no purpose in shooting .410 rifled slugs in a rifled .45 caliber barrel. The slug never gets a "seal" coming out of the barrel and they tend to "wobble" downrange.
