Potentially several per planet. My starting system has 6 planets. I just stumbled upon a settlement on the starting planet, so I highly doubt there's only one per system (the chances of me finding it if there were are astronomically low). I'm assuming there's at least one per planet, and likely several.

If you got your hopes up of owning multiple different settlements on different planets, I have bad news for you. Only one settlement can be managed much like a freighter.
