There was a clip on reddit a while ago of a player scav spawning with a red and immediately being gunned down. So it "can" happen, the odds of which i have no idea. Friend of mine spawned with it on him as a scav once, last wipe. I feel like I have spawned with one at some point. I don't think it's possible.
