All right, I found the answer to my question. For anyone having the same issue: The handkerchief is being removed automatically after the boss fight in Leper's Smile. However, it is not needed. All you need is the perfume, for which you have to pass the perception check.

Haste can help you get to the queen faster and delay poison communal to stop the con drains. One of the best things is just to memorize the path and buff everything before you talk to the mission start lady. The timer doesn’t start until after you speak with her.
